Prescription drug monitoring programs (state-implemented programs that contain all patient-specific controlled substance dispensing records from outpatient commercial pharmacies) have been demonstrated as effective tools to identify opioid prescribing patterns that are associated with opioid-related misuse, abuse, overdose, and death (risky prescribing patterns). By analyzing PDMP data, the Idaho Department of Health and Welfare (IDHW) will be able to identify characteristics of the Idaho PDMP associated with best outcomes and maximizing the utility of PDMP policy and implementation. Furthermore, the data will enable IDHW to detect patterns, trends and identify prescriber demographics associated with risky-prescribing and inform a targeted response to educate these providers, including primary care physicians. For this project a retrospective analysis of the Idaho Prescription Monitoring Program data from 2013 -2018 associated with opioid prescribing patterns will need to be completed. This will include cleaning (the process of identifying and correcting incorrect and missing data) the raw data, adding additional variables, including Morphine Milligram Equivalents (MME), and analyzing the clean data sets. Risky prescribing patterns will need to be assessed using methodologies described in the CDC Prevention for States Indicator Toolkit (to be provided by the Department), including simple linear regression of quarterly risky prescribing patterns.

In addition to the 2013-2018 PDMP dispensing data, the 2016-2017 (2018 data when available) utilization data will need to be cleaned and linked (by a unique identifier: hashed DEA) to the corresponding cleaned PDMP-dispensing data sets. These linked datasets will be analyzed to identify characteristics of provider-patient lookups and the integration of “Gateway” (PDMP integration software for the electronic health record), associated with prescribing patterns.

The result shall be provided in a written report (25 page max) and select indicators (provided by the department) shall be provided in datasheets and analyzed by select demographics and time periods.

Indicators include, but are not limited to:
1. Opioid analgesics per 1,000 state residents
2. Patients receiving more than an average daily dose of =90 morphine milligram equivalents (MME) of opioid analgesics
3. Multiple provider episodes for prescription opioids (five or more prescribers and five or more pharmacies in a six-month period) per 100,000 residents
4. Patients prescribed long-acting/extended-release (LA/ER) opioids who were opioid-naive (i.e., patients who have not taken opioid analgesics in 45 days).
5. Patient prescription days with overlapping opioid prescriptions

The contractor is expected to work approximately ¾ full time per week, including 2-4 hours per week consulting with DPH staff on progress, results, etc. All deliverables will be completed by January 31, 2019.

The contractor shall be responsible for providing their own computer and a licensed copy of SAS 9.3 or newer. The contractor will work at an off-site location but will be required to meet, preferably on-site but it could be via a remote phone/screen sharing tool, with DPH staff weekly for approximately 2-4 hours. The contractor should have four plus years of relevant experience and also be knowledgeable and able to work independently using SAS 9.3, data management software (e.g., Microsoft Excel), and work independently. Preferred education is a four year college degree or equivalent technical study in statistics, epidemiology or similar field.
