With direction from the Tennessee Department of Health (TDH) and/or the Regional Healthcare Coordinator (RHC) the VPC will work to address these disparities through identified projects and programs. Job Responsibilities · Collaborate with relevant government agencies, non-government organizations (NGOs), and community organizations. Recruit new CMS-17 and support service members to healthcare coalitions. · On-board newly recruited agencies to respective healthcare coalitions. · Increase engagement to community-wide meetings/councils/coalitions by inviting vulnerable populations contacts to attend the most applicable meeting for their focus-area mission. The following are examples of community-wide meetings that may be applicable based on county: Healthcare Coalition meeting Health Council meetings Healthcare Facilities Local Emergency Planning Committee Drug Coalition meetings · Compile a community resources list for response agencies and healthcare facilities to utilize during emergencies. This list will be developed, considering factors such as age, disabilities, language barriers, socioeconomic status, and medical needs. Examples of resources in this list: Durable Medical Equipment providers Oxygen tank vendors Mobility aid & assistive devices vendors Food banks Housing/sheltering NGOs · Biannually review & share the resource list and update point of contact and other vital information. · Link existing community resources to drive resiliency within whole community. · Maintain healthcare coalition datasets for the purposes of verification and compliance with coalition policies and procedures. Information and documents such as: Inventory and preventive maintenance logs ICS Verification Memorandums of Understanding for coalition purchased assets Healthcare Resource Tracking Systems (HRTS) Offer monthly HRTS training to coalition members and external stakeholders. Assist RHC with troubleshooting user accounts when necessary. Patient Bed Matching (PBM) Provide technical support to all stakeholders on PBM to reduce data disparities and inconsistencies. Escalate unresolved issues to PBM staff. Offer monthly PBM training to applicable stakeholders. All other coalition-related activities Offer all other training related to assigned healthcare coalition as designated by Regional Healthcare Coordinator (RHC) monthly.
