Description:

Description: Vendor shall provide established content expertise paired with project management to conduct the following activities as part of the continuing implementation of a systemwide change effort, which includes the National Center on Advancing Person-Centered Practices and Systems Technical Assistance (NCAPPS TA).

Working with the NCAPPS TA, vendor shall develop a work plan, coordinate and facilitate all aspects of a person-centered practice organizational assessment, coordinate and facilitate the system redesign and implementation, and coordinate and facilitate the implementation of person-centered planning training with ND Department of Human Services (DHS) leadership.

Vendor shall work collaboratively with NCAPPS TA partners, consumer/advocate groups, agency partners, and DHS core person-centered planning work group.

Vendor shall have the capacity and ability to provide person-centered communication and facilitation recognizing individuals/project teams needs and preferences. This may include removing barriers for participation, including the coordination of accessible meeting/work spaces and using technology/solutions that meet the need for all members participating in project work groups, meetings, and steering committees.

Vendor must have internal capacity of content expertise in behavioral health, developmental disabilities, vocational rehabilitation, aging and physical disability, brain injury, and children and family services.

Work Plan shall include:

• Definition of the overall program strategy, drivers, expected outcomes, and success criteria
• Creating a project governance structure with roles and responsibilities.
• Coordinating and facilitating project meetings, acting as a point of contact for stake holders, including: DHS staff, consumer/advocate groups, and NCAPPS TA point of contact(s).
• Informing change management approach and strategy. Assisting in the development of a timeline for the implementation of the assessment and the strategy.

Initial Project Planning:

• Implement and manage all project activities.
• Develop written plan of action and timeline for the assessment of person-centered practices across DHS.
• Work with project teams to conduct assessment of person-centered practices and policies for each division within DHS.
• Develop a structured and consistent approach that will document and track the assessment of the following: a review of administrative rule, century code, and other policies as identified.
• Compile results of the assessment of person-centered practices and provide written documentation of the findings.
• Facilitate a discussion among stakeholders to develop the vision in moving forward with organizational change.
• Develop a written plan action and timeline for implementation and timeline.

Communication Planning:

• Coordinate and facilitate all interactions and meetings between stakeholders, including: DHS staff, consumer/advocate groups, and NCAPPS TA point of contact(s).

Vendor shall coordinate a system-wide assessment process that will include:

• Internal and consumer interviews to develop a deep understanding of how each DHS division delivers services today, from the consumers point of view.
• Work in collaboration with NCAPPS TA group to develop and conduct a consumer survey for each DHS division.
• Compile results of interviews and survey in a report format.
• Identify Moments of Truth where DHS should focus its greatest attention to ensure an exceptional and consistent experience.
• Identify areas to improve the experience through industry best practices and voice of the consumer.
• Develop tools like customer experience narratives and customer insight reports.
