The specific services to be provided under this PAQ are unknown at this time, including the total number of deliverables, hours, and cost. The total estimated amount that the State may expend for this PAQ is $50,000. All estimated numbers herein are estimates only, are provided for informational purposes only, and such estimates are not intended to reflect any guaranteed usage for this PAQ. The State of Missouri shall not guarantee any minimum or maximum amount of the contractors services that may be required under the PAQ. If the need for services arises, the contractor and ITSD will agree on the scope, timing, and cost of each deliverable before the commencement of work. Background: WebSurv is the MDHSS statewide comprehensive disease registry for collection of data for reportable communicable diseases including hepatitis, sexually transmitted disease, tuberculosis, vector borne conditions, etc. WebSurv is part of the Missouri Health Strategic Architectures and Information Cooperative (MOHSAIC) System. MOHSAIC is comprised of an Oracle (version 10.2) database backend and numerous applications which are primarily written in Microsoft.NET. MOHSAIC uses Oracle role based security and tracks data deletions and modifications through audit procedures. Several Department of Health web applications, as noted in the diagram, make up the MOHSAIC System and are all focused around one centralized common infrastructure. When working within the MOHSAIC structure, care must be taken to ensure that changes to a MOHSAIC application are self-contained and do not affect other MOHSAIC applications. See diagram(s) for more information. Attached is MOHSAIC System WebSurv Diagram and additional detailed information about the WebSurv system including detailed description of the .NET applications in the system. PROJECT APPROACH AND SCOPE OF WORK: This Critical Support PAQ is being requested under the Statewide Information Technology (IT) Consulting Services contract. This PAQ shall not exceed $50,000 in cost. The contractor will report to the ITSD Project Manager and will provide staff with sufficient knowledge and skill to resolve WebSurv production issues that are classified as “Critical” or “Major” using the definitions below. Work orders will be prioritized by ITSD and a preliminary cost estimate will be provided by the contractor. ITSD will authorize all issue work to be performed. The ITSD project manager will make appropriate resource assignments to complete the work. Work on these issues will commence within a designated time frame based on the category. Each issue report assigned under this PAQ will require a resolution to the problem, an incident report explaining the cause of the problem, and what was done to correct it, and updated documentation and actual cost. On-site consultants shall observe Missouri State holidays. No out-of-state travel is required on the part of the state or the vendor for completion of this project. Consultants may be exchanged under this PAQ without modification of the PAQ upon agreement of both parties. The state reserves the right to review resumes and accept/reject proposed consultants. The state shall be invoiced on a monthly basis for the costs incurred for critical and major issues/problems closed during the month for which applicable deliverables have been documented and accepted using an ITSD Project Deliverable Acceptance Form.
