Description of Specific, Results-Oriented Tasks

The State shall provide the Contractor an example template for each survey questionnaire. On an annual basis, the State shall provide to the Contractor the required changes that need to be made to the survey questionnaires. The Contractor must make the changes and provide the State a copy for approval prior to the Contractor mailing the surveys. The final survey questionnaires will be very similar to the samples provided. In some instances, the only changes that need to be made are the calendar year dates on the survey.
The Contractor shall print, address, and mail the surveys. A stamped return envelope must be provided with each survey at the Contractors expense. All surveys will use the same size business return envelopes and outgoing envelopes. Outbound envelopes shall be #10 white window envelopes with black ink on one side. The reply envelopes shall be #9 white envelopes with black ink on one side. Surveys will be 8 ½ X 11 white 20 lb. bond paper with black ink. Six are two sided, the rest are one sided, all are folded. A cover letter is required for the Deer Hunter Observation survey. The State shall provide the mailing lists for each survey in a format compatible with the Contractor. The mailing lists will include complete name and address information which is obtained from license applications. Addresses provided will be sampled from the most recent hunting season. The Contractor should run an address correction through the post office before the survey is mailed. Mailing lists for small game and waterfowl will be provided for the preseason postcard prior to the hunting season. The State aims to sample a fixed proportion of hunters; accordingly, the number of hunters included in mailing lists will fluctuate annually according to the number of licenses sold.
Mailing using standard presort is sufficient. Contractor will need an “Address Service Requested” endorsement. If surveys are not delivered, they need to be returned and re-mailed to forwarding address.
Some surveys may require the Contractor to mail an additional one page insert with the survey questionnaires. The State will inform the Contractor which surveys should include the insert and will
provide the insert to Contractor. Contractor should quote a price per insert (e.g. 4 cents per insert).
The Contractor shall provide the ability for the survey respondent to fill the survey out over the internet. The Contractor shall include the web address on the survey questionnaires. The internet surveys do not need to be ADA Section 508 compliant. Each survey will have a unique ID number to prevent paper/internet duplicate responses. Each survey does not require a separate website. A portal can be provided that allows access to each individual survey section.

Quality Assurance
The State shall periodically communicate with the contractor via email or telephone to address questions, concerns, or progress. The State shall proof the required changes made to the survey questionnaires prior to being mailed by the Contractor. The State shall provide the mailing list of names and addresses

Deliverables
The contractor will be required to provide the following deliverables:
(a) Provide the State a draft of the questionnaire, reflecting updates to calendar dates and any additional requested changes by the State, for approval prior to the Contractor mailing the surveys.
(b) Provide the ability for the survey respondent to fill the survey out over the internet and include the web address on the survey questionnaires.
(c) Enter the data from the returned survey questionnaires in an MS Access database according to the States specifications.
