Scope of Work

Component 1: State SCSEP Program

A. Overview
STATE is soliciting proposals to carry out the state SCSEP services to individuals age 55 and older in designated counties in North Dakota.

B. Eligible Host Sites
The following organizations are eligible to apply to serve as host sites:

? Non-Profit Entities;
? Tribal Government Entities; and
? Other Governmental Entities.

C. Service Eligibility
Eligibility criteria for state SCSEP participants are:

? Be 55 years of age and older;
? Have an income of no more than 125 percent of the federal poverty level;
? Be unemployed and not job-ready; and
? Reside in North Dakota.

D. Service Priority
Priorities for selecting individuals for participation are:

? 65 years of age or older;
? Have a disability;
? Have limited English proficiency or low literacy skills;
? Reside in a rural area;
? Are veterans (or eligible spouses of veterans) for purposes of the Jobs for Veterans Act;
? Have low employment prospects;
? Have failed to find employment after using services provided under the Workforce Innovation and Opportunity Act (WIOA); and
? Homeless or at risk for homelessness.
F. Service Locations
Successful offeror shall provide the services at locations as follows:

1) Meets local, state, and federal HIPAA requirements; and
2) Accessible to individuals with disabilities.

G. Service Availability
Successful offeror shall provide the services throughout the contract term.

H. Service Provision
Successful offeror shall at a minimum:

1) Recruit and Select Participants
a) Develop and implement methods to recruit and select eligible participants to assure maximum participation in the program;
b) Use income definitions and income inclusions and exclusions for SCSEP eligibility to determine and document participant eligibility;
c) Develop and implement methods to recruit minority populations to ensure proportional representation in the service area as defined in Section 2.02 Scope of Work; and
d) Develop and implement strategies to recruit applicants who have priority of service as defined in Section 2.02 Scope of Work.

2) Assess Participants
a) Assess participants at least twice per 12-month period; and
b) Use assessment information to determine the most appropriate community service assignments (CSAs) for participants.

3) Establish Individual Employment Plans (IEP)
a) Establish an initial goal of unsubsidized employment for all participants;
b) Update the IEP at least as frequently as assessments occur (twice per 12-month period);
c) Modify the IEP as necessary to reflect other approaches to self-sufficiency, if it becomes clear that unsubsidized employment is not feasible; and
d) For participants who will reach the individual durational limit or would not otherwise achieve unsubsidized employment, include a provision in the IEP to transition to other services.

4) Select Community Service Assignments (CSA)
a) Base the initial CSA on the assessment done at enrollment;
b) Select only designated 501(c)(3) organizations or public agencies as host agencies;
c) Develop procedures to ensure adequate supervision of participants at host agencies;
d) Develop procedures to ensure safe and healthy working conditions at CSA through annual monitoring and
e) Develop procedures to offer the opportunity to participants a virtual CSA.

5) Recertify Participants
Recertify the income of each participant at least once every 12 months, or more frequently if circumstances
