Bidder must have an ITPS Master Contract with DES for the ITPS_08215_02. IT Business Analysis and/or ITPS_08215_04. IT Project Management. 14.2. Bidders Proposed Consultant must have a minimum of three (3) years experience as an Agile practitioner and coach within a software development environment. 14.3. Bidders Consultant being proposed for this engagement must have provided design, development and implementation services for Agile development frameworks and process within a software development environment on similar large, complex Projects within the past three (3) years. 14.4. Bidders Consultant must have provided services such as consultation, technical guidance and knowledge transfer (coaching) to support the adoption of the agile development methodology on similar large, complex Projects within the past three (3) years. 15. Requirements of Bidder 15.1. (M) The Bidder must be willing to sign a Work Order with terms substantially similar to those in Attachment 2: Proposed Work Order/Contract. 15.2. (M) The Proposed Consultant must be available to work in the Olympia area. Provide a written commitment that the Proposed Consultant by Bidder will be available for the anticipated term of the Work Order and will be the onsite person to perform the contracted work. 15.3. (M) Bidder must state whether the Proposed Consultant, or any officer or employee of the Bidder, or subcontract or any staff of a subcontractor of the Bidder, is or was an employee of any agency of the state of Washington during the twenty-four (24) months preceding the Response due date of this Work Request. If yes, Bidder must provide such individuals name, employing agency, job title while with the agency, and separation date. 15.4. (M) Bidder must state whether the Bidder or any subcontractor has contracted with the state of Washington during the past 24 months. If yes, Bidder must indicate the name of the agency, the contract number, project description and/or other information available to identify the contract. 15.5. (M) Bidder must state whether the Bidder has had a contract terminated for default in the last five (5) years, and describe such incident. Termination for default is defined as notice to stop performance due to the Bidders non-performance or poor performance and the issue of performance was either: (i) not litigated due to inaction on the part of the Bidder, or (ii) litigated and such litigation determined that the Bidder was in default. a) Submit full details of the terms for default including the other partys name, address, and phone number. Present the Bidders position on the matter. HCA will evaluate the facts and may, at its sole discretion, reject the proposal on the grounds of the past experience. b) If no such termination for default has been experienced by Bidder in the past five (5) years, so indicate. 15.6.
