Description :

This statement of work (SOW) is for Application Development and Support, for new and existing development of functionality to support the Florida Department of Health, Medical Quality Assurance Licensure System and all affiliated intranet and internet services for the Division of Medical Quality Assurance, Bureau of Operations

The DOH will conduct a criminal history record check, including fingerprinting, on the consultant assigned by the Contractor prior to the consultant commencing work for the Department. The Department retains sole discretion as to whether the results of the criminal history record check will result in the Contractors employee being disqualified from performing services pursuant to this agreement.

The Contractor must have, at a minimum, the following experience:

5.1 Oracle Related Experience

5.1.1 Six years of experience with Oracle products inclusive or Oracle Database Server, Oracle Application Server, Oracle Streams, Oracle Data Guard, Oracle Grid Control, and Oracle RMAN.

5.1.2 Six years of experience with PL/SQL programming.

5.1.3 Six years of experience with Oracle relational databases, OLTP and DSS, serving in high performance, service-oriented, multi-tiered environments.

5.1.4 Six years of experience with Source Code Control System (SCCS) for the management of internal code changes to Oracle Forms/Reports, Oracle objects, and scripts.

5.1.5 Six years of experience with development and maintenance of Perl scripts and UNIX shell scripts.

5.2 SQL Related Experience

5.2.1 Six years of experience with MS SQL products inclusive of MS SQL 2008.

5.2.2 Six years of experience as an MS SQL administrator inclusive of data design/modeling, monitoring, and performance tuning.

5.2.3 Six years of experience with SQL programming inclusive of stored procedures, functions, triggers, DTS packages, scheduled jobs, and linked servers.

5.2.4 Six years of experience with SQL relational databases, OLTP and DSS, serving in high performance, service-oriented, multi-tiered environments.

5.2.5 Six years of experience with configuration management (SourceSafe administration) for SQL database objects.

5.2.6 Six years of experience with database design tools including ERWin and ERStuido.
