Position: Advanced Data Base Analyst (DBA) Qty: 4,000 hours Rate: $75.00 per hour Term: July 1, 2018- June 30, 2019 State Contract: 80101507-SA-15-01 5. CONTRACTOR QUALIFICATIONS: 5.1. All Contractor staff assigned to this agreement must possess, at minimum, the following qualifications and experience: 5.1.1. 5 years of experience as a Database Administrator (DBA) using Structured Query Language (SQL) Server. 5.1.2. 2 Years of experience using SQL Server Reporting Services or crystal reports. 5.1.3. 3 years of experience related to performance tuning in SQL Server. 5.1.4. 3 years of experience using Microsoft Transact-Structured Query Language (TSQL) writing complex queries using multiple joins from multiple sources and/or calculations. 5.1.5. 2 years of promoting changes through the database development life cycle. 5.1.6. 2 years of experience in SQL Server Integration Services (SSIS). 5.1.7. 3 years of experience in the management of multiple projects, user support, and documentation of operational needs. 5.1.8. 2 years of experience in relational database design, indexing, normalization, and denormalization. 5.1.9. Associates degree or above. 6. CONTRACTOR RESPONSIBILITIES: 6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified: 6.1.1. Provide development, maintenance, and delivery of database code for tuberculosis (TB) Consultation, Incentive and Enablers and TB Surveillance projects as defined during Joint Application Requirements (JAR) sessions and/or joint application design (JAD) sessions through database administration and documentation for storage, data design and modeling, monitoring, performance turning, and PI/SQL in PRISM as directed by the Department. 6.1.2. Provide technical written documentation to HIV, TB and STD Section Administrators or designees describing any implemented changes to PRISM in regards to PRISM configuration management, patch management and version control of administration, patching of MSSQL and web application servers, and complete Department Change Management process as it relates to PRISM and relates to functionality of the system as directed by the Department. 6.1.3. Develop SQL queries, scripts, and job processes to meet grant, project-specific, stakeholder, and other partner data for routine business practices within PRISM as directed by the Department and maintain these files on the local network where previously developed queries, scripts, and job processes are stored. 6.1.4. Submit data to local, state, and federal stakeholders by designated deadlines as defined in the data request and respond to findings in accordance with federal and state STD reporting guidelines in audit reports from state and federal stakeholders as directed by the Department. 6.1.5. Provide an audit report quarterly of the type and frequency of PRISM help tickets completed by the contractor along with timeframes needed to correct system errors or implement userdefined requirements and enhancements as directed by the Department. 6.1.6.
