PURPOSE: This scope of work is for vector borne diseases including Zika Epidemiology and Data Quality Manager for the Division of Disease Control and Health Protection in the Bureau of Epidemiology within the Zoonotic and Vector borne Disease Program. Contractor will provide these services to the Florida Department of Health (Department). 5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE: Contractor staff assigned to this agreement must possess the following minimum qualifications and experience: 5.1.1. Two years of experience in formal training or work in the epidemiology field. 5.1.2. One year of experience in biostatistical analysis work. 5.1.3. Two years of experience in conducting interviews. 5.1.4. One year of experience in providing training to staff. 5.1.5. Two years of experience in Microsoft Office Professional including Excel, Word, PowerPoint and Outlook. 5.1.6. Completion of Master in Public Health (MPH) with an emphasis on epidemiology and/or biostatistics. 5.1.7. Two years of experience in epidemiology and Date management experience including completion of an epidemiology project with statistical analysis of large data set. 5.1.8. Completion of one course each related to SAS, SPSS, and statistical analysis software package. 5.1.9. One-year experience producing written reports summarizing findings from statistical analysis. 6. CONTRACTOR RESPONSIBILITIES: 6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified: 6.1.1. Basic and extended data queries of vector borne diseases including Zika data to assess for data completeness and accuracy as directed by the Department. 6.1.2. Basic data queries are completed within one business day as directed by the Department. 6.1.3. Extended data queries are completed within one business week as directed by the Department. 6.1.4. Reports will be prepared and emailed to the appropriate requester, once completed as directed by the Department. 6.1.5. A copy of requested data reports will be maintained in the “I” drive for at least one year as directed by the Department. 6.1.6. Provide at least one electronic data tool (query, report, etc.) for county health department staff related to vector borne diseases including Zika case follow-up as directed by the Department. 6.1.7. Provide vector borne diseases including Zika extended data queries related to symptoms, risk factors and other key factors as directed by the Department. Extended queries must be provided in the requested format (line list, table, graph, etc.). Standard requests must be prepared and delivered within one business day (highly complex requests may take longer); high priority requests must be prepared and delivered the same day. 6.1.8. Retrieve data, convert into requested format for electronic delivery to requesters outside of the Department within one week of the request for routine data requests.
