The term of this SOW shall begin on July 1, 2017, or on the date which the order is issued, whichever is later. It shall end at midnight, Eastern Time, on June 30, 2018. The State of Floridas performance and obligation to pay under this purchase order is contingent upon annual appropriation by the Legislature and the performance of the Contractor. The DOH will conduct a criminal history record check, including fingerprinting, on the consultant assigned by the Contractor prior to the consultant commencing work for the Department. The Department retains sole discretion as to whether the results of the criminal history record check will result in the Contractors employee being disqualified from performing services pursuant to this agreement. The Contractors staff must have, at a minimum, the following experience: The following outlines the expected educational level and experience requirements for the contractor, to support the needs of PRISM projects: 1. 5 years of experience as a Database Administrator (DBA) using Structured Query Language (SQL) Server 2. 2 years of experience using SQL Server Reporting Services or crystal reports. 3. 3 years of experience related to performance tuning in SQL Server. 4. 3 years of experience using Microsoft Transact-Structured Query Language (TSQL) writing complex queries using multiple joins from multiple sources and/or calculations. 5. 2 years of promoting changes through the database development life cycle. 6. 2 years of experience in SQL Server Integration Services (SSIS). 7. 3 years of experience in the management of multiple projects, user support, and documentation of operational needs. 8. 2 years of experience in relational database design, indexing, normalization, and denormalization. 9. 1 year of experience as a DBA using PRISM 10.
