Client: Department of Health.

Description:

The Department will conduct a criminal history record check, including fingerprinting, on the consultant
assigned by the Contractor prior to the consultant commencing work for the Department. The
Department retains sole discretion as to whether the results of the criminal history record check will
result in the Contractors employee being disqualified from performing services pursuant to this
agreement.
5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE:
5.1. The Contractor must possess, at minimum, the following qualifications and experience:
5.1.1. Project Planning, Organization, Implementation:
5.1.1.1. 6 years of experience with database navigational and analysis tools.
5.1.1.2. 6 years of experience of documented application development expertise in the following
technologies and products: object-oriented and object-relational design and
development, web technologies, XML, HTML, MS SQL, Microsoft .Net (VB.Net, ASP.Net),
MS Visual Studio.
5.1.1.3. 6 years of experience with modifying, developing, designing, and maintaining .NET
applications to meet design and change specifications.
5.1.1.4. 6 years of experience with MS versioning tools (Microsoft TFS, Microsoft Visual Studio
Team Services).
5.1.1.5. 6 years of experience with Visual Basic, Classic ASP. SQL server and Oracle databases.
5.1.1.6. 6 years of experience with maintaining technical documentation for all development
tasks.
6. CONTRACTOR RESPONSIBILITIES:
6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified:
6.1.1. Provide object-oriented and object-relational design and development, web technologies, XML,
HTML, MS SQL, Microsoft .Net (VB.Net, ASP.Net), MS Visual Studio 2003 and 2005 on a weekly
basis as directed by the Department, including, but not limited to:
6.1.1.1. Modify, develop, design, and maintain .NET applications;
6.1.1.2. Design model objects (tables, views indexes); and
6.1.1.3. Provide analytical services for system incidents and enhancements.
6.1.2. Process tickets to correct system errors or implement user-defined requirements for each
defined defect/enhanced ticket as directed by the Department within the timeframe directed by
the specific ticket request. Processing tickets includes, but is not limited to:
6.1.2.1. Process defect tickets to correct system errors within one (1) to two (2) business days;
6.1.2.2. Process release tickets to manage user requirements within the timeframe directed by
the specific ticket request. The response (due) date of tickets assigned will range from
one (1) hour to more than one (1) week;
6.1.2.3. Process change tickets to implement system changes on a weekly basis;
6.1.2.4. Provide analytical services on systems to implement user-defined requirements; and
6.1.3. Deliver code to perform system functions as defined in user requirements for each defined
defect/enhancement ticket as directed by the Department within the timeframe directed by the
specific ticket request. Delivering code includes, but is not limited to:
6.1.3.1. Provide analytical services on systems to identify operational efficiencies;
6.1.3.2. Assist with collecting system requirements with the business program staff; and
6.1.3.3. Document system technical objects, business requirements, business processes, and
system processes.
6.1.4. Test database code created to satisfy system fixes or user defined requirements as directed by
the Department for each defined defect/enhancement ticket within the timeframe directed by
the specific ticket request. Testing database code includes, but is not limited to:
6.1.4.1. Test application
