This PAQ is a follow up to the PAQ completed in Oct 2013. Office of Administration/Information Technology Services Division (OA/ITSD/DESE) shall provide oversight for this PAQ. The Department of Elementary and Secondary Education (DESE), Office of Special Education (OSE) requires enhancements for the Regional Professional Development Centers (RPDC) web based application. This PAQ is to enhance the applications current functions, reports, security, and to meet the business needs and OA/ITSD/DESE standards. Based on analysis of business enhancement requests and OA/ITSD/DESE development standards the goals and objectives of this project are to: •Modify code to meet security roles. •Create new and modify existing reports using Visual Studio 2012. •Modify current key functions and displays in the RPDC application. •Modify previous code and ensure new code is meeting OA/ITSD/DESE standards. •Provide 30 days of post implementation support. PROJECT APPROACH AND SCOPE OF WORK: This firm fixed price deliverable PAQ is being requested under Statewide Information Technology (IT) Consulting Services Contract. The Contractors PAQ Response must include a Project Overview, Resources, Approach/Methodology, Testing Plan, Change Control Plan, Issue Tracking Plan, Quality Assurance Plan, Firm/Fixed prices for each deliverable, and a Draft Project Plan which must specify the tasks to be completed for each deliverable and the time frames in which the tasks shall be completed. A finalized detailed project plan with work breakdown structure must be provided within two (2) weeks of a mutually signed PAQ. Status reports shall be submitted weekly and upon request to the OA/ITSD/DESE Project Manager, or a designee. Status reports shall include but are not limited to: key activities conducted during the given reporting period, key activities planned for the next reporting period, and documentation regarding any outstanding issues. See attached ITSD Status Report template. Security & Confidentiality - All materials (including code, tools, documentation and data) provided pursuant to this PAQ shall be deemed confidential. All consultant(s) shall be required to sign an OA/ITSD Confidentiality and Information Security Agreement form.
