Four (4) years of relevant community health worker, community outreach, or health education experience may be considered in lieu of degree. · Two (2) years of prior experience in a CHW role; supervisory experience is a plus · Three (3) years of experience in community outreach, health education, or in a role representing or supporting local community agencies · One (1) year of progressive leadership experience relevant to a Population Health role is preferred · Two (2) years of prior experience working in a Community Health Worker or Social Work model preferred   Call 502-379-4456 Ext 100 for more details.
